Terms of Engagement between Nexus HealthHub and the Patient

These terms of engagement form the contract between Nexus HealthHub, a collaborative clinic between Dr. Indra and Mark Kingham, Chiropractor ("the Practice"), and the patient ("you" or "the patient"). They set out the boundaries of the relationship between the practitioners and the patient. Please authorize your consent by filling in the information below.

ABOUT CHIROPRACTIC CARE

Chiropractic care focuses on diagnosing and treating neuromuscular disorders, with an emphasis on treatment through manual adjustment and/or manipulation of the spine. Chiropractors aim to reduce pain and improve the functionality of patients as well as educate them on how they can account for their own health via exercise, ergonomics, and other therapies to treat back pain.

Chiropractic treatment can help with a range of conditions including but not limited to back pain, neck pain, pain in the joints of the arms or legs, and headaches. Treatment typically involves hands-on spinal manipulation, which chiropractors use to restore mobility to joints restricted by tissue injury caused by a traumatic event, such as falling, or repetitive stress, such as sitting without proper back support.

The philosophy of chiropractic care is to maintain proper alignment of the body's musculoskeletal structure, particularly the spine, to enable the body to heal itself without surgery or medication. By addressing the biomechanics and structure of the body, chiropractors can help relieve pain, improve function, and support the body's natural ability to heal itself.

Functional medicine treats each person as an individual, aiming to get to the root cause of health problems. It’s a systems-oriented approach that assesses the whole patient and engages both the patient and the doctor in a therapeutic relationship. Functional medicine addresses the entire person, not just an isolated set of symptoms.

Practitioners at the Practice spend time with patients, listening to their histories and examining the interactions of genetic, environmental, and lifestyle factors that influence long-term health and complex chronic conditions. This approach offers a powerful clinical model for the assessment, treatment, and prevention of chronic disease. Each individual is considered unique, and personalized nutrition and lifestyle programs are recommended, recognizing that outcomes may vary even among patients with similar health issues.

Functional Medicine GPs

Our functional medicine GPs primarily deal with chronic health problems. They can work alongside other medical and health care professionals with your consent. Our standards of professional practice in medicine are governed by the GMC Code of Professional Practice.

Chiropractors

Chiropractors at Nexus HealthHub focus on diagnosing and treating neuromuscular disorders through manual adjustment and manipulation of the spine. Our chiropractors aim to reduce pain and improve patient functionality. They educate patients on how to manage their health through exercise, ergonomics, and other therapies to treat and prevent back pain. Chiropractic care helps with conditions such as back pain, neck pain, joint pain, and headaches. By ensuring proper alignment of the musculoskeletal structure, particularly the spine, chiropractors aid the body's natural ability to heal itself without surgery or medication. Our chiropractors integrate with other treatments to provide comprehensive care through personalized assessments and treatment plans.

THE PATIENT UNDERSTANDS AND AGREES TO THE FOLLOWING:

  • Continue with current treatment and keep your primary care provider or NHS team involved. Any risk and safeguarding information will be communicated as needed.

  • You must be registered with an NHS GP for routine and acute medical care and consent to us contacting your GP regarding any aspects of your case. If deemed a threat to yourself or others, we have a duty to contact your GP without your consent.

  • Provide a basic patient summary and medication list before your initial appointment.

  • Contact the practice for clarification if unclear about changes to your nutritional plan, supplements, or medications.

  • Contact the practice if you wish to continue any specified supplement program beyond the agreed period to avoid potential adverse reactions.

  • Recording consultations without written permission from both the Doctor and the Patient is prohibited.

  • An administration fee may be charged for ordering, processing, and interpreting products and/or functional medicine testing.

  • Implementing coaching services is entirely at your own risk. You assume all risks of participation in lifestyle changes or choices made in connection with coaching sessions and take full responsibility for your well-being and decisions before, during, and after coaching.

  • Notify the practice if working with other healthcare providers during your care.

  • Consent to case discussions within the practice’s Collaborative Care meeting to support your health plan.

  • Understand that while following evidence-based medicine and NHS guidelines, we may also step outside this guidance with your consent.

Cancellation Policy

  • Payment and forms must be received 48 hours before your appointment, or it will be canceled unless prior notification is given.

  • 48 hours' notice of cancellation is required, or the full appointment cost will be charged.

  • For program care packages or memberships, failure to provide 48 hours' notice will result in no refund or rescheduling of the appointment. With proper notice, appointments can be rescheduled within the program timeframe.
